DISCLAIMER: If you have turned 18 since you began your care at Denova, please contact us to set up a new portal for you as an adult. Portals for minors expire when patients turn 18. If this is an emergency call 911. If you are currently experiencing a crisis please call the crisis hotline at 602-222-9444. If for any reason the patient is unable to keep the scheduled appointment it is the responsibility of the Patient, Parent, and/or Legal Guardian to call Denova staff, at least twenty-four (24) hours in advance, to cancel. If the Patient fails to cancel their appointment at least twenty-four (24) hours in advance, a $50 no show fee will be applied. This fee must be paid prior to being seen for any Medical, Dietetic, Psychiatric and/or Counseling appointment. Note: All AHCCCS patients are exempt from this fee.
