Case Management is a supportive service offered to our patients enrolled with Medicaid who need extra help with the basic necessities of life. This might include access to safe housing, healthy food, and proper clothing. When these needs are met, you can better focus on your health and well-being.

Denova Collaborative Health is at the forefront of providing comprehensive case management services such as household, food, and clothing assistance, to our AHCCCS patients, both adults and children. While our psychiatry, mental health, and primary care clinicians can help with bodily needs, our case managers are in charge of supporting you in all other areas of your life that encompass your well-being (resources for housing, food, clothing, childcare services, and more). Our team is not only experienced but also possesses an in-depth understanding of community resources.

Household Assistance Program
